Benzodiazepine as a pain medication in emergency settings
Emergency departments frequently come across a wide range of patient with multiple types of presentations. The most common feature among them is pain, which may be acute or chronic. Acute pain can be due to appendicitis, or a gun shot wound, or acute exacerbation of a chronic pain. Chronic pain is usually in the cases of rheumatic arthritis, or cancer patients. Regardless of the type of the pain, the medical personnel are always needed to decide which pain medication is needed for a particular patient.Understanding of individual drugs, their mode of action, their potency, and their possible interactions with the personââ¬â¢s condition or with other drugs is necessary to prevent any adverse consequences of prescribing. There are many pain killer drugs now available in the market. With the rapid progression in the pharmaceutical industry, there is a larger pool for the health care workers to prescribe from, and specification for all types of conditions. Apart from the administration of the pain killer drugs, there are many legal aspects that need to be considered as well. The recognition of patients who may be developing addiction, or are using hospital as a source of addiction is very important which may require certain corrective and legal measures. Apart from the use of such painkillers, there are many studies and researches that point to the role of various psychotropic drugs in the management of pain.While the association may seem sparse on the up front, patients with pain are frequently agitated and worried, and may require some ââ¬Å"calming of the nervesâ⬠. In such patients the use of psychotropic and anti anxiety drugs is considered a good option. There are many researches that now support this theory, and claim that an emergency setting can respond better with appropriate use of these drugs. It is however important to take into consideration the type of pain for which the patient presents with.While acute pain patients may not have a large chance of developing a dependence on the drugs, it is the chronically ill patients that are of more concern. Patients of cancer and arthritis etc. are prescribed with chronic pain medication in accordance with the severity of the pain. Most of these patients are given prescriptions, which may not be maintained or fully monitored. While in the hospital admitted cases, the identification of addiction and dependence patterns can be easily diagnosed and dealt with, the same is not true for patients who are living in homes and receiving their medications their.To address this problem the government again introduced various measures to help control the dispensing of such drugs, and periodic evaluation of such patients at outpatient settings. (Sorelle, 2002) Researchers in studies have shown that many times the patients are given the pain medication very late or not at all. However, there are many factors that complicate the situation as simply patient comes, patient receives, and patient leaves. The time it may take for various preliminaries, the history taking and the final decision about whether the patient does need medication are among the various factors that can affect prompt medicine giving.The studies have pointed out the lack of information and lack of management of pain in the systematic manner. These studies therefore point out to the need of a good method of understanding and relieving pain. (Sorelle, 2002) Current pain management strategies recommended include the following: â⬠¢ Introduction of low doses of drugs initially and gradually increase to reach the optimum effect for the patientâ⠬â¢s pain relief. â⬠¢ Combined drug therapy. This helps in reducing the doses of the drugs, and thereby prevents side effects of one drugâ⬠¢ Continuous analgesia, which is now being used widely in emergency settings. â⬠¢ Use of behavioral methods along side pharmacological interventions to increase response. (Feinberg, 2004) OPIOID ANALGESICS IN THE MANGEMENT OF PAIN: Centrally acting opioids analgesics are used widely for the remedy of pain in the clinical settings. Tramadol is among the new line of opioids narcotic agents that are used for moderate to severe pain. It acts both as a weak opioids agonist and as an inhibitor of monoamine neurotransmitter reuptake.Both oral and parenteral forms have shown good results in the management of pain. (Feinberg, 2004) NSAIDS IN THE MANAGEMENT OF PAIN: The second most common line of drugs used in this group of medication is the COX 2 inhibitor drugs.These drugs have shown at least similar efficacy in the management of pain when compared to COX non specific drugs. Parecoxib sodium for example, has similar effects to non specific nsaids in the injectable form. These drugs have shown a superior activity to morphine for most measures of analgesic efficacy, and have a prolonged duration of action. (Scott et al,2002) USE OF PAIN SCALES FOR THE DIAGNOSIS OF PAIN SEVERITY Many researches are now questioning the value of the visual analog scales used widely to monitor the level of pain.Bodian et al, (2001) carried out a research to identify the relation between the changes of analgesic intravenous doses with the documentation on the VAS charts by the patients. The study also aimed to find out if watching previous VAS scores by the same patient h ad any influence on the later scoring of the pain. Bodian was able to conclude that ââ¬Å"when pain is an outcome measure in research studies, grouping final VAS scores into a small number of categories provides greater clinical relevance for comparisons than using a full spectrum of measured values or changes in value.Seeing an earlier VAS form has no apparent influence on the later values. â⬠(Bodian et al, 2001) PAIN MANAGEMNET IN CHILDREN: Pain management in children especially neonatal kids was not much debated until in the recent years. However, there are many recent methods that are used to alleviate pain in the recent pediatric settings. These include prevention of pain, where the pain is treated prophylactically. The identification and proper assessment of the pain is another crucial element in the pain management in children.The treatment of pain while initially considered to be limited to only providing analgesics, has recently evolved to include physical methods su ch as massages, transcutaneous electrical nerve stimulation, and behavioral methods such as hypnosis, distraction, preparation and rehearsal. (Zempsky and Schechter, 2003) Current child pain management heavily relies on the information received from the parents, and uses them during the pain procedures as a source of comfort to the child.The introduction of painless analgesia technique is another method of inducing cooperation from the patient. And reduction of anxiety, a very important feature in a child is another very important area in the management of pain. (Zempsky and Schechter, 2003) Benzodiazepines are used very widely as sedatives in children with successful results. However, the only problem associated with it is the development of tolerance to it. In such children withdrawal symptoms cause severe distress and the child may express irritability, ataxia, aggression, hallucinations, and twitching.Other symptoms include inconsolable crying, agitation, vomiting, anxiety, hype ractivity, convulsions, tremors, jitterness, poor feeding gagging. The form used in children is midazolam, and is used for a variety of purposes. Their indications include sedation, amnesia, anticonvulsant, and anesthesia. (Bennett, 2001) If benzodiazepines are given to such children for more than 3 days, then these should be weaned off slowly. If the child exhibits withdrawal symptoms, then diazepam, a less intensive benzodiazepine can be given until the effect wears off.(Bennett, 2001) The pain scales used in children include the Oucher Scale, Faces Scale, Poker Chip Tool, Colored Analogue Scale, the Pain Thermometer, Visual Analogue Scale, and the McGill Pain Questionnaire respectively. The most common drugs used in pediatrics include opioids and NSAIDS. Included in the pain management techniques are behavioral approaches such as distraction etc. The current evaluation scales have improved considerably since the last two years, and have improved the outcomes of the pediatric pain management as well.(Zempsky and Schechter, 2003) THE USE OF ADJUVANT ANALGESICS IN THE MANAGEMENT OF PAIN. With increased understanding about the various modalities of pain, there are now novel and newer methods introduced in medicine to reduce pain in patients. The use of adjuvant drugs is used to enhance the efficacy of the analgesia and to improve the tolerance to pain. Drugs such as antidepressants, antihypertensives, local anesthetics, and CNS stimulants are widely used as adjuvants to control analgesia and to improve patientsââ¬â¢ experience of pain.

RICHARD BRANSONââ¬â¢S VIRGIN GROUP ââ¬âProblemsââ¬â Although certainly notorious, Richard Bransonââ¬â¢s Virgin Group has experienced several problems. In the short-term, brand overextension has become a threat since the company has entered multiple markets. Brand overextension leads to damaging the comprehensive Virgin brand. Since the Virgin brand itself is the companyââ¬â¢s greatest asset, protecting the brand image is critical. The Virgin brand image has a connotation of value and lifestyle. Due to many joint ventures, licenses, and alliances with other companies, the objective of the ââ¬Å"Virgin experienceâ⬠could be destroyed.Furthermore, as Virgin becomes more global and more adaptive to individual cultures, the customer experience will certainly vary, again leading to brand overextension. For example, although the United Kingdom may condone Virginââ¬â¢s racy and edgy advertisements, other parts of the world, as our group testifies, views such materia l quite explicit and even shocking. Thus, while Virgin enters different markets, negative impacts can arise, hindering Virginââ¬â¢s brand. A second short-term problem is Virginââ¬â¢s financial information that is fragmented, hard to locate, and even interpret.With the companyââ¬â¢s different alliances and joint ventures, questions about the financial health of the Virgin group were being asked. Thus after examination, Virgin faced financial challenges that led to negative cash flow. This was mainly due to the Bransonââ¬â¢s approach of stand-alone basis where the most profitable companyââ¬â¢s revenues financed new market entrances. Despite some profitable areas of Virgin, many did not generate cash. Therefore, as Virgin grows, a more systematic approach must be implemented to create synergies between businesses. Long-term problems also exist for Virgin.First is Bransonââ¬â¢s close affiliation to Virgin. What happens if the public tire of his exploits? What will happ en to Virgin when Branson 1 leaves? An implication of these questions is that Virgin needs Branson. Virginââ¬â¢s strong brand is fortified by Bransonââ¬â¢s own personal values. A second long-term problem for Virgin is the companyââ¬â¢s little formal structure and management system. Although this structure presently works for Virgin, allowing the company to be viewed as entrepreneurial and innovative, in the long-term creativity must be managed at Virgin. Company Analysisââ¬â After analyzing Virgin through various analytical frameworks including internal/external analysis, SWOT analysis, and value chain analysis, we have discovered the importance of Virginââ¬â¢s strong brand and diversification strategy. In sum, Virginââ¬â¢s strongest capability is the companyââ¬â¢s strong brand. The brandââ¬â¢s foundations of entrepreneurship and eccentricity result in Virgin being a strong customer-oriented company. Since the Virgin brand is so strong, the company has been able to diversify into many new markets.Diversification for Virgin is made possible by joint ventures and strategic alliances that are ultimately less risky. Thus the Virgin brand leverages Virginââ¬â¢s resources. Furthermore, Bransonââ¬â¢s values are revealed in the Virgin company. The brand image, which is the motor of the company, results in innovative start-ups that keep Virgin both successful and sustainable. ââ¬âAlternativesââ¬â To continue Virginââ¬â¢s success, we have identified several alternatives. First, Virgin could go public; thereby gaining additional capital that could be used to fund new start-ups.The disadvantage with this alternative is that Virgin would be forced to abide by formal accounting rules, which Branson states he does not favor. Furthermore, Virginââ¬â¢s brand could become diluted since the image would move towards a more traditional stance. 2 A second alternative is that Virginââ¬â¢s management could designate Bransonââ¬â¢ s su ccessor and prepare that person to take on the task of CEO. The advantage of this alternative is that the fate of Virgin is not attached to Branson alone. This could help separate Branson from the Virgin brand that allows Virgin to flourish as a company, and not merely an extension of Branson himself.Additionally, new leadership could prove beneficial for Virginââ¬â¢s long-term success, and namely deciding Virginââ¬â¢s structure. However, Bransonââ¬â¢s absence could also weaken the brand in the short-term. Since the public easily identifies with Bransonââ¬â¢s traits as loyal and fair, customers could perceive Virgin negatively. To thwart such initial negativity, Virgin could slowly implement the new successor, allowing him or her to also revel in the spotlight alongside Branson. Thirdly, Virgin could dispose of unprofitable companies.This consolidation would result in money saved and the creation of a more formal organizational structure. Therefore, across the board, Vir gin would be able to have a more uniform decision processes. Consolidation would also reassure investors since the corporate structure would be less complex and financial data more readily available. The motives for diversification are growth, risk reduction, and profitability. If Virgin cannot achieve those goals through the diversification strategy, then there are no reasons why Virgin should keep unprofitable companies.Focusing on related industries would then give Virgin expertise; and thereby creating both value for shareholders and exploiting economies of scale. Furthermore, Virgin could allocate resources among the firms more efficiently, giving Virgin better future strategy formulations. 3 ââ¬âRecommendationsââ¬â One must analyze Porterââ¬â¢s essential tests that that include the attractiveness test, cost-ofentry test, and the better-off test, to determine whether a diversified strategy has a sustainable competitive advantage.Because Virginââ¬â¢s strong brand i s firmly embedded in the companyââ¬â¢s corporate structure, the attractiveness test reveals that manyââ¬âthough not all (example is the unprofitable Virgin Money)ââ¬â of Virginââ¬â¢s diversified companies are attractive. Secondly, costof-entry must be considered. For Virgin, many entry costs are feasible, because profitable Virgin companies support the new start-ups. And thirdly, the better-off test shows that new companies within Virgin must be profitable and add value.Although many new companies do not add financial benefits, the new companies do add brand value. Thus the answer to the question whether Virgin is a successful diversified company is clearly yes. Overall the diversification works quite well. However, we do recommend that Virgin exit the unprofitable industries and consolidate. This would be beneficial in the short-term as capital immediately becomes available. Furthermore, this would also help Virgin in the long-term as only profitable companies remain, allowing Virgin to increase revenue.In looking at Virginââ¬â¢s long-term success, we recommend that Virgin continue its short-term strategy of further developing expertise in key areas. Thus, long-term market saturation would be avoided, while also maintaining Virginââ¬â¢s strong brand awareness. Despite Bransonââ¬â¢s strong presence, we also suggest that Virgin slowly implement a successor. Virginââ¬â¢s brand is about personalityââ¬âindeed the company needs a strong focal point. 1. Did the exterior of the worship facility add to the overall religious feeling of the visit? Describe your first impression as you pulled up to the building. Did the architecture lend itself to worship? Add specific details to support your answer. I always said I would visit a mosque one day. I live in a high Muslim community and have always been invited to go with one of my peers. The exterior of the building was designed right out of what I would describe as old time desert days. The building had domes that came to a peak sort of like an ice cream on top of a cone. My first impression was that this was surely a design that was distinctive from the other buildings of worship like Christian, Baptist, and Catholic churches, or even halls for Jehovah Witnesses. As I came closer to the doors I felt overwhelmed by the actual size of the building but inside looked much like home. Inside it was simple, there were restrooms, and places for women to tend children, many windows, Qurans, and a large drape right down the middle separating carpeted space one side for men and one side for women. 2. Describe the nature of the worship facility service you saw. This will probably be the longest section of the report, so be as thorough as possible. For example, what was the overall religious program? What was the theme of the message for that day? Were there any rituals that struck you as different from your own? Elaborate Once service started I wasnââ¬â¢t sure what to expect. Before we were able to sit in the service we first had to wash our feet and hands. There were no programs but there were men who help sort of like the ââ¬Å"mcâ⬠over the crowd. Service started with a prayer in which nearly everyone except me knew. Although I tried my hardest to keep my eyes close during the prayer for the sake of this paper and my curiosity I open my eyes to see all except a few small boys not speaking aloud the prayer. During this prayer everyone is in the position on his or her hands in knee as to sort of like submit while during the prayer. Once the prayer was over we resumed position on ourà knees. Next was the message, I could not help but notice that there was no women in our section, instead through I barely visible colorful sheet the women were in their own service and prayer. The message today wasnââ¬â¢t really a message at all but instead a teaching of the Quran. Before the reading of the text spare Qurans were passed around to members who wanted one. I opted to take one only to find out the entire book was written in another language. During the reading the text was read as is in the language was written in, and later after the reading translated for others. The text he spoke about interesting enough was about Jesus and his actual role Muslims believe he played. Although Christian believe Jesus played a huge role, Muslims believe that not only is he not the Child of God but just another of God prophets. I also learned that they believe that Jesus did not do many of the healing described in the Bible. Accompanied with scriptures I cannot read nor remember Muslims also believe their text to be older and more accurate than that of the Bible. The only ritual that I think struck me is the fact that women were not able to congregate with us instead they were behind the curtain. I would later find out the reason. The washing of hand in feet were different for me and me being conscience of my feet I felt a little uneasy about the situation. 3. What was your overall reaction to the service? Was it positive or negative? Did any members of the congregation talk to you, and if they did, what was the nature of the conversation? My overall reaction was interested I would say. I had a positive experience and a lot of people approached me after the service about my inquiry of Islam. I was told I came during a good sermon and they hoped I learned from the experience, if I was coming back and also what I thought about the whole thing. I told them I like the experience and I was honestly doing this for a class and that my interest peaked most when I actually walked inside the building. I asked about the curtain and was kind of token back with the answer. I was told that the curtain is used to hide the women during the service. Because women are appealing to the eye it is sometimes hard to concentrate on worship and this simply isnââ¬â¢t tolerated. I could see the point of view about a ââ¬Å"time and place for everythingâ⬠but I think if aà person will is that of a higher being one would not engage in deeds. I also notice that after, women mingled amongst each other, but younger women and kids mingled amongst each other. I also asked a guy why made him follow this religion, and he told me culturally where his parents are from this is like tradition. Similar to religion and myself being passed down he had experience the same thing. He also mentioned that although he was sort of given this religion he believe in it whole-heartedly. He said that many of the other religions text had been altered many times and the Quran the last standing oldest religious text was untampered.

Irony can be a very effective rhetorical method, pouring contempt on an idea or principle much more harshly than a straightforward attack. Irony sneaks up behind you and ambushes you. It is a good way to rhetorically attack problems and situations that are right in front of you and that many people might want to defend. With irony you can attack but others will not realize you are attacking until it is to late (Horn 76). This is what Twain has done in The Notorious Frog. For ma ny who picked it up, it would take some time to realize it was all a joke. When they did realize it was a joke, they would be shocked and begin to really think about what Twain meant. This is an especially militant form of ironyââ¬âsomething Twain truly excelled at. People will always take advantage of one another. Gullibility is omnipresent. A lesser writer might have been tempted at the end of the story to reveal the whole thing to be a joke. But Twain is a rhetorical master. He realizes that consistency is everything in rhetoric: if you shift out of the voice you appear disjunctive and lose the full effect. Criminology - Essay Example The labeling theory was developed from the interactionist perspective of criminology. Interactionists believe that a person does not become a criminal on their own. Interactionist theories "assume that everyone has the potential to violate the law and that criminality is not an innate human characteristic," according to Schamlleger (2005, p. 245). While social constraints are the primary cause of crime, this alone does not cause someone to act unlawful. Instead these deviant behaviors must be learned and reinforced, which is why the process of socialization is an important factor in determining if someone is going to become a criminal. This means that interactions with society, peer groups, family, the education system, and other social groups play a prominent role, as these interactions provide the individual with morals, values, and a slate of norms in which to adhere to. In addition, this theory believes that a person's position in society will influence their involvement in crime , with those in higher positions in society less likely to partake in criminal behaviors. The labeling theory helped criminologist differentiate between primary and secondary deviance. An offender's initial act of deviance is considered primary deviance. However, continuing these acts is considered secondary deviance, especially if these acts occur as a result of being around other deviant individuals (Schmalleger, 2005). For example, while someone may initially rob a gas station as their primary deviance, secondary deviance may come from being sentenced to jail, meeting fellow criminals, and engaging in additional deviant behavior with them that would not occur if the individual was not associated with them. However, the individual does not have to have direct interaction with other deviant individuals for it to be considered secondary deviance. The action of simply being labeled a criminal can result in secondary deviations (Schmalleger, 2005). Noting and studying labeling in regards to secondary deviance is important, because the negative labels put on an individual e ventually become internalized. The individual labels himself as deviant, and therefore performs deviant behaviors to fit these labels (Schmalleger, 2005). In the 1960s and 70s, criminologists began to question the labeling and interactionist theory. It was during this time that there were a lot of social problems that were causing criminologists to question their loyalty and support to these and other earlier proposed theories. Some of these social problems included the civil rights movement, which gave blacks and whites equal rights, and the war in Vietnam, which was hugely unpopular. Criminologists began to question how they could support theories and interpret laws that discriminated against people and reinforced or caused human suffering. It was at this time that things in the world of criminology began to shift, as a new perspective, Marxist criminology, began to develop. This criminology theory strongly questioned the labeling theory and other theories before it. It was founded upon the writings concerning capitalism by Karl Marx. He was an individual who believed that capitalist societies were divided into two social classes, a small, rich, elite and a large, poor, working class.
